Pathway Partnership Programme

The Pathway programme offers hospitals a better way to help people who are homeless.

We work with the NHS to help spread and develop our successful model of enhanced care coordination for people who are homeless and other excluded groups. Working with NHS partners we want to reach more people who will benefit from the service.

This is why we have developed a partnership programme; to partner with more NHS providers to deliver high quality care to people who need it most.

Pathway was founded to improve health care for homeless people and others deeply excluded from society

We support Clinical Commissioning Groups, Local Authorities and Hospital Trusts to work together to help the most vulnerable in our society. We help our partners to set up integrated, cost-effective NHS homelessness teams in hospitals.

Embedding a Pathway team in your area will bring real change to the lives of people experiencing homelessness.

We hope we can work with you to improve the lives of some of the most vulnerable people in our society. I look forward to welcoming you to our growing national network of services and passionate and dedicated homeless health professionals in hospitals across the UK.

By joining Pathway's network you are also joining a national movement for change and making visible commitment to tackle the worst health inequalities.
